Judges' breed health monitoring

The purpose of this form is for judges to provide feedback to The Kennel Club on any visible health or welfare concern, or signs of discomfort in any dog(s) entered under them. The information provided via this form will be passed on to the dog health group which monitors and compares trends across the breeds.
