Judge’s Equipment Incident Report Form Please list all specific incidents including the equipment involved, such as a dog coming off the top of the dog walk, etc. There is no need to record anything you would consider “normal” (i.e. Agility faults such as knocking down a pole, missed contacts, banking or paddling the long jump, etc.) Feel free to explain the circumstances in more detail if you feel this helps to put the incident into context. The details submitted with this form will be sent to both the agility team at The Kennel Club and a representative of the Equipment Panel for the Agility Liaison Council.
