Judge’s Equipment Incident Report Form This form is for Judges to submit incidents involving agility equipment. If your not the Judge but wish to submit an incident report. please do so using the shows Incident Book. You can do this at the show or up to 14 days afterwards by emailing the show secretary. Please list all specific incidents including the equipment involved, such as a dog coming off the top of the dog walk, etc There is no need to record anything you would consider “normal” (i.e. Agility faults such as knocking down a pole, missed contacts, banking or paddling the long jump, etc.) Feel free to explain the circumstances in more detail if you feel this helps to put the incident into context The details submitted with this form will be sent to both the agility team at The Kennel Club and a representative of the Equipment Panel for the Agility Liaison Council You may be contacted separately if the incident warrants further research or follow up by either The Kennel Club or any research The Kennel Club is involved with so, please provide up to date contact details If you have a course plan, photos or video you'd like to provide please send these via email to agility@thekennelclub.org.uk Your name Your email address Your preferred contact details (ideally a phone number) Please confirm you were the judge for the incident you are reporting Select an option Yes No Date of incident Kennel Club Show name Ring No Class No Height category No. of dogs in the class Weather conditions at the time of the incident Including suns position, glare or shade if relevant Ring surface e.g. sand, grass, astro Please include any comments on surface such as mud or bare patches etc if relevant Surface condition in the ring at the time of the incident Please list the obstacle(s) involved in the incident Make/brand of the equipment (if known) Please summarise the incident that occurred, providing any details that help with context, such as obstacle sequence, dog's line, handler's path, weather/ground conditions, dog(s) involvement/experience, any relevant external factors. Any images, course plans or video you'd like to provide should be sent via email to agility@thekennelclub.org.uk.
